Competition Registration Form

Complete details for each answer must be provided for the entry to be accepted.

For an explanation of categories please refer to the Competition Rules/Categories documents on the previous page.

For Duets, only one form needs to be submitted for the duet.  Do NOT have each person do a separate form.

For groups, each individual member is not required to submit a separate form.  The form for each group entry should be submitted by the conductor/teacher and contain the names of all group members.
